Nursing Theories

Betty Neuman’s System Model focuses on mental health nursing. Neuman proposed the model of holistic nursing based on stressors in the patients’ environments. Neuman expresses that things such as environmental stressors can negatively affect the patient externally and internally. Neumans major concepts described in her theory involve the holistic approach, open system, environment, client system, health, stressors, degree of reaction, prevention as intervention and reconstitution (Wasaya, at el., 2021). Neuman expresses that she believes that the human body is in a constant back and forth between balance and imbalance. In this model, the concept is not directly aimed at only the patient but the patient as a whole including family, group, society and the environmental factors that affect everything as one. Neuman believes that the environmental factors that affect the patients health and the natural line of defense in these coping mechanisms can depend on how flexible their line of defense is (Wasaya, et al.,2021). These “line of defenses” affect the patient’s ability to manage acute health problems. If these natural defenses are damaged, the outcome of health management is projected to be worse. These lines are made up of five different characteristics; physiological, psychological, social, developmental and spiritual domain (Wasaya, et al.,2021). Neuman believes that health is at a constant changing pace of wellness and illness. In her eyes, prevention of illness and decrease in wellness is a way to prevent these lines of defenses from becoming weaker. Keeping the wellness of the patient’s health can positively impact the patient’s illness. In relation to prevention, there are three levels of prevention; primary, secondary and tertiary. The primary prevention is before the patient’s body even reacts to a stressor, it is essentially decreasing stress all around in their lives. The secondary line is the management after the patient has had a stressor happen, where treatment begins. Tertiary is managing and preventing other occurrences from happening (Wasaya et al.,2021).

Hildegard Peplau Theory of Interpersonal Relations is also focused on mental health and psychiatric nursing. Her theory is defined as initiating and maintaining interpersonal relationships between the nurse and patient. When a strong, interpersonal relationship is formed, there is essentially a better health outcome for these patients. Peplau states that following these characteristics leads to a good relationship; effective communication, therapeutic nature from the nurse, the nurses strive to learn about the patient’s choice/ wishes and the overall professional relationship between the nurse and the patient (Wasaya, et al.,2021). Peplau describes nurses needing to have these six characteristics to ensure these relationships can be formed; stranger, resource, reaching, leader, surrogate and counselor (Wasaya, et al., 2021). These interpersonal relations happen through three phases, the orientation phase, identification and exploitation which then results in a positive outcome.

These two theories play a role in my personal practice because I am always having to deal with environmental stressors. I am a Labor and Delivery Nurse and so patients are often stressed out due to their environment by things such as unwanted visitors. It is my job as the nurse to eliminate those stressors and to provide the best care possible to my patients. I have to be the bad person to tell those unwanted people to leave. In the end, if my patients are less stressed, then the outcome of their care is better. As a nurse, every time there is interaction with a patient there should be an interpersonal relationship initiated and built. This forms trust between the nurse and patient. These patients are relying on us, we as nurses have to build that bond and trust to ensure the patients are getting the best care and outcomes as possible. We are our patients number one advocate.
