Soraya is a 35 y/o Caucasian female who presented to the clinic complaining of “extreme nervousnessâ€.
During the interview, she begins to talk rapidly and gets very tense easily. She claims that all her life she has worked hard to be a very successful person. In her recent job, she has performed really well, and she was promoted to a new position seven months ago. Her new responsibilities include being in charge of supervising one hundred employees. She claims that she has multiple meetings online and in-person meetings during the day. When she gets home and starts thinking about all the things she has to at work the day after, which makes feel more “nervousâ€, "fatigued" and it is interfering with her sleep time. She only gets about 4 or 5 hours of sleep. She went to her primary care provider, and did her annual check up and laboratory work and the results were unremarkable. She denies drinking alcohol, smoking, or using any drugs.
During the last 6 months, she claims that, she gets palpitations, has muscle tension, sweaty hands, shortness of breath at work at least 4 times a week, and that these symptoms are also experienced at home almost every day. Her anticipation to having these symptoms make things “worseâ€. These feelings have been affecting her performance at work and it is also putting a strain in her marriage. She wants help from the PMHNP to manage her symptoms. She is willing to try “anything.
