“Leadership is the vision and mission to be something more than average” (Khoshhal & Guraya, 2016). Being a leader requires a person to be knowledgeable, trustworthy, flexible and confident. As a leader everyone looks to you for the answers to any problem. You must be well versed in your area of expertise and confident enough to act on that knowledge. Leaders need to be flexible. Plans change, things happen, and you must adapt to these changes. Being trustworthy is vital to your success as a leader. Your staff need to know that they can trust in your decision making and trust the information you give them to act on.

In terms of transformational leadership, I relate most to the “individualized consideration” dimension. I do not think that you can take a “one size fits all” approach to leadership. We all have our own beliefs and through processes and leaders should consider each person individually. This dimension focuses on “mentoring each employee to help them reach their full potential” (Boamah et al, 2018). I do see myself as someone who thinks positively and encourages others to strive to do their best. I enjoy mentoring and teaching others.

The TEACH value I think would most benefit my work environment is Teamwork. I truly believe that teamwork makes the dream work. The Social Services office I work in can be hectic at times and there are days where I honestly would not make it through without the help of my coworkers. Having a combination of seasoned, knowledgeable nurses and younger nurses provides a good mix of resources. We are able to come together with the ideas and knowledge we each possess to help our patients. At the end of the day we are there to give the best possible outcomes to our patients and it usually takes a team to make that happen.

Interdisciplinary teams are very prevalent in my clinic. We utilize, nurse’s, CCHT’s, healthcare providers, social workers, and dietitians in dialysis. Each are needed to establish a good foundational base for the patient to have a healthcare team to benefit from the best healthcare outcome possible. Each have a different role and specialty that is required to meet the needs of the patient in dialysis for example, the social worker helps the patient more with financial needs, as well as, presenting different benefits to the patient about insurances, helping with ride assistance and so forth. While the dietician educates patients on renal diets, foods to avoid and encouraged foods. The use of binders and certain labs such as phosphorus and albumin. The health care providers role is to meet the medical needs of the patients.

The nurse’s role is one that provides patient care, educates that patient on health conditions and needs, and is an advocate for the patient. As noted within our text, “Advocacy is the act or process of pleading for, supporting, recommending a cause or course of action” (ANA, 2015a, p. 41). This is true of all settings as a nurse. While the role of the CCHT is to report findings to the nurse and help reinforce education. It is through the interdisciplinary team that we can provide functional, safe, and best care practices to our patients providing the with the best healthcare outcomes possible.

Some gaps that can be found within the teams at times are availability of certain members of the teams. The healthcare provider is there one day a week, the dietician maybe twice a week, the social worker perhaps once every two weeks or so. In this case the nurse stands in to meet the patients’ needs at that given time. The nurse advocates for the patient by contacting the other members to get answers for the patient or meds from the healthcare providers. In my opinion the nurse can be seen as the glue that holds the team together.
