The behaviors of a population can put it at risk for specific health conditions. Studies of human characteristics have shown that those with similar health risks can be targeted for population health improvement.

Choose two groups from the health conditions listed below.
Tobacco use
Alcohol abuse
Opioid addiction
Obesity
Heart disease
Pancreatic cancer
Begin your project by briefly discussing why you have chosen the groups.
Research and discuss descriptive characteristics that are typically associated with the groups you have chosen.
Thoroughly research and discuss at least five different types of epidemiological tools used to study the two groups.
Finally, conclude with detailed recommendations for improving the health of those who develop the conditions you chose.

Chart audits are widely used, but the information in them varies. For example, checking for proper signatures on all forms and orders can be used universally, while daily recording of medication refrigeration temperature will only be used in some areas.

Select a minimum of three and a maximum of four indicators for measurement in your quality improvement program.

For each indicator, include the following information:

Indicator: Describe the indicator.

Rationale: Explain why this indicator is suitable for the high-risk area you have chosen.

Measurement: Describe and explain how you will measure this indicator.

Finally, explain how these indicators taken together cover all aspects of quality in the high-risk area.

The Impact of Chronic Illness
Identify one person from the illness group you chose in Week 1. The person should not be a patient at the facility in which you work. You can use friends, family members, or coworkers. Do not use the person’s name in the paper but only initials. Administer the questionnaire you created in Week 1 to that person. Compile the data and analyze the responses to better illustrate where this person, his or her family, and friends are in relation to accepting the diagnosis in relation to the standard health or illness definitions. The analysis should also include coping skills, treatment, and support aspects of the illness. Identify how this will direct care plan development for the chosen illness group.

Compile a report of your interview with the questions you created, the responses you received, your analysis, and your interpretation of how it will affect planning care for the group in a 4- to 5-page Microsoft Word document.
