Constitutional: Decrease appetite and 5% weight loss; denies fever
ENT: Denies sore throat or drainage
Lungs: Occasional dry cough at night
Heart: Mid sternum chest burning intermittently
Abdomen: Epigastric burning and tenderness, denies constipation or diarrhea
