Air travel in the 1970’s and 1980’s was considerably different from what we experience today. A series of high profile crashes and equipment failures plunged the industry into a period of reassessment and redesign. On March 27, 1977, one of the most significant disasters in flight history occurred at the airport in Tenerife, Canary Islands. Through a series of improbable events, two Boeing 747 jumbo jets collided on takeoff. Five hundred and eighty-three people were killed, making the Tenerife accident the worst aviation disaster in world history. Through analysis, discussions with survivors, and transcripts of the cockpit recordings, safety scientists and engineers were able to recreate the events of that day.
Watch the PBS (2006) film, The deadliest plane crash [49:44 minutes], for a step-by-step analysis of the circumstances that led to this disaster.
While you view the case study, keep in mind Reason’s (2000) article, “Human error: Models and management”

and use the Swiss cheese model to keep track of the “holes in the defenses.”
