Dr. Janette Simon is an OB/GYN physician with over 20 years of experience in the field. She has focused her practice on the care of young women with AIDS. She has dealt with extremely difficult, and often heart-wrenching situations over the years but is currently dealing with something new. A patient of hers for several years, Lucy Camp, is 16 years old and is the mother of a healthy 2 year old son. Dr. Simon has been called to the ER to see Lucy with complaints of severe abdominal pain. Lucy has had AIDS for over 4 years. She has been in and out of the hospital with pneumocystis pneumonia for the last 6 months and has not been able to fully recover.
Lucy is pale, weak, her skin lesioned and thin. She looks in worse condition than Dr. Simon has ever seen her. She states that she is doing much better than she was. She says she doesn’t hurt everywhere like she was and she has been able to eat without vomiting since yesterday but she has been having these terrible pains just in her abdomen today. She relates to Dr. Simon that not eating has actually been a good thing because it allows she and her husband to spend less money on food for themselves so there is more for her son. Most of their money goes to pay for her medications and medical bills.
Dr. Simon examines Lucy and determines she has an ectopic pregnancy complicated by severe internal bleeding. She needs surgery immediately. When Lucy hears the news that she is pregnant, she cries. She cries hysterically and doesn’t seem to comprehend that she has an ectopic pregnancy. Dr. Simon explains again. This time Lucy seems to comprehend the implications of the ectopic pregnancy. She stops crying and calmly asks Dr. Simon what would happen if she did not have surgery. Dr. Simon explains that is not an option because this problem is life-threatening. She could die.
Lucy calmly states, "I refuse the surgery." Dr. Simon senses someone behind her and sees Lucy’s parents have entered the room. Her mother states, "Lucy, you have to have this surgery. Dr. Simon, tell her she has to have the surgery."
