AHRQ Culture of Safety Survey
Read and follow the directions very carefully. This is a multi-part process for completing what is required for this discussion.
1) Go to the Agency for Healthcare Research and Quality (AHRQ) web site, the surveys on patient safety culture page. Review the information on the SOPS page. Watch the brief marketing video for a quick overview of the SOPS.
2) On the left-hand menu of the SOPS web page, choose the most appropriate setting for your current workplace (hospital, medical office, or nursing home). It looks like this:
3) Download or print the survey form, the Survey items and composite measures, and user guide. This can be done from the site, or the links are included below for the most common setting options. If you work in a different type of setting than what is listed, pick the survey that is the closest option. The user guide is for additional understanding, but not intended for the type of self assessment that you will be doing for this assignment.

4) Fill out the survey as it applies to your workplace. You only need to fill out the scored items. You do not have to submit this survey but will need the results in order to move forward in preparation for this discussion.

Nursing Home Survey: Complete sections A – E
5) Open up the items and composite measures document. You will see which questions fall into each composite. Tally the scores for your workplace in each composite and put them in order. Note that the negatively worded question scores are reversed. (See example for how to score)

6) Go to the Survey of Patient Safety Databases page and locate the most recent database for the survey you chose. (Links provided below for ease.) View the executive summary and compare your organization’s weakest and strongest composites. Provide a summary of this comparison in the discussion board along with your personal reflection on what you found.
