CLASS NAME: PRIMARY CARE ACROSS THE LIFESPAN III
LESSON 1: CARDIOVASCULAR AND RESPIRATORY DISORDERS
ASSIGNMENT 11.1: Heart murmurs
Introduction
The focus of this assignment is heart murmurs.
Assignment Guidelines
As an FNP, you will diagnose heart murmurs. This usually causes panic in parents, so understand what you are hearing. Describe a situation in which you diagnose a heart murmur on a pediatric patient. Include grade/intensity, timing in cardiac cycle, location on the chest where the murmur is the loudest, radiations or transmission, quality, duration and pitch. Use Box 38.2 (Auscultatory Findings–Innocent versus Pathologic Murmur) and Table 38.5 (Common Innocent Murmurs) from your Burns’ Pediatric Primary Care textbook. Name the murmur and follow-up recommendations.

Lesson Introduction
This week reflects on different cardiovascular and respiratory disorders in our pediatric population. First, learn how to identify whether a heart murmur is innocent or pathologic. Next, touch on signs and symptoms of specific congenital heart diseases. Moving to the lungs, differentiate between the multiple respiratory conditions and how to treat them and utilize Pri-Med for further education on bronchiolitis.
Lesson 2 reflects on disorders of the gastrointestinal and genitourinary systems. Review how to perform a thorough history and physical assessment involving the abdomen. Identify multiple gastrointestinal and genitourinary conditions that might affect the pediatric population, along with recommended evidence-based treatment. Lastly, recognize the use of probiotics and their role in the health of our pediatric population today.
The first lesson of this week focuses on cardiovascular disorders in the pediatric population. Review heart murmurs and how to differentiate between an innocent heart murmur and a pathologic heart murmur, and review signs and symptoms of specific congenital heart diseases.
This lesson also focuses on the lungs. Identify multiple upper and lower respiratory disorders seen in the pediatric population and recommend evidence-based treatment. Revisit Pri-Med to learn more about how to recognize and treat bronchiolitis, an inflammatory bronchial reaction in young children and infants.
Learning Outcomes
At the end of this lesson, you will be able to:
• Describe a heart murmur and identify if it is innocent or pathologic. (CO1)
• Identify signs and symptoms of specific congenital heart diseases. (CO8)
• Identify multiple upper and lower respiratory disorders seen in the pediatric population and recommend evidence-based treatment. (CO8)
• Recognize signs of bronchiolitis and recommend evidence-based treatment. (CO7)
Note: You will find a “(CO#)” after each learning objective stated in this course to indicate which Course Outcomes are being addressed. The Course Outcomes are outlined in the Syllabus.
Learning Materials
Read the following in your Burns’ Pediatric Primary Care textbook:
• Chapter 38, “Cardiovascular Disorders”
• Chapter 33, "Atopic, Rheumatic, and Immunodeficiency Disorders" (Asthma, pp. 523–539)
• Chapter 37, “Respiratory Disorders”
Read the following in your Pediatric Physical Examination: An Illustrated Handbook textbook:
• Chapter 8, “Heart and Vascular Assessment”
• Chapter 9, “Chest and Respiratory Assessment”
These readings help to recognize and identify cardiovascular disorders, as well as to recognize and properly treat multiple upper and lower respiratory disorders seen in the pediatric population.
