Module Aims:
To develop an understanding of the impact that long term conditions have on patients’ physical and psychosocial well being.
To develop awareness of the health care resources available to provide
appropriate and informed nursing care and support for those with long-term
conditions.
Learning Outcomes:
Understanding of the role of the nurse in meeting care needs of patients with long- term health conditions, applying knowledge relating to assessment data, in order to plan safe, appropriate and individualised care.
To be able to formulate appropriate plans of care for patients with long-term conditions, using integrated care planning and/or care pathways, recognising the importance of the roles of others in multi-professional care.
To be able to analyse the use of national service frameworks, government policies, self-management and current evidence which informs the management of patients with long- term conditions.
To be able to apply a legal and ethical framework and a non-discriminatory approach to analysing situations from practice.

ASSESSMENT TASK AND STUDENT GUIDANCE
The summative assignment for this module is a 3000 word care study of a patient with a long-term condition, in whose care you have previously participated, in an acute or community setting. The essay will be an exploration the role of the nurse in meeting care needs of a patient with a long- term health condition or helping to control their symptoms, applying knowledge relating to assessment data in order to plan safe, appropriate and individualised care.
The essay must include the following elements:
Identification of the pathophysiological effects of the long-term condition that the individual lives with, explained in such a way as to demonstrate your own understanding.
A discussion about how the individual preferences, strengths, abilities and needs of the person were identified, through assessment, including evaluation of any model used.
A discussion about the process of agreeing with her/him a care plan that is based on her/his personal needs, including drawing upon the support of other Health professionals.
Justification of planned interventions, utilising published literature from a range of sources
A review of care against published guidelines, such as a relevant National Service Framework; against NICE guidelines or against guidelines from health charities; campaign groups or the King’s Fund.
The care study will include other elements, depending upon what you identify as important. One option is discussion of the ethics of a particular situation, if relevant to your essay. You may choose to introduce your essay with relevant statistics about Long-term conditions or a particular LTC.
Planning care should attempt to meet all the needs of a person but for this assignment, you should limit focus on just one or two of the person’s problems, since your discussion of problems should not be superficial.
Although you might meet your chosen person in an acute setting, there must be some focus on meeting on-going, long-term needs. It is quite acceptable to make part of your discussion about the process of agreeing a care plan that was focused on preparing for discharge from hospital.
