As a reminder, each week your assignment will comprise of a vital component of your Final Program Evaluation Report final project. As such, you are encouraged to ensure that you use the feedback you receive each week to make the necessary revisions for your final report.

Also review the Week 4 Tables Template Download Week 4 Tables Template. You must use the Evaluation Stakeholder Workgroup table from this template to complete your paper.
Clarity of methods and data in a report is key to enabling the audience to understand the
information presented and thus act upon the evaluation results. Focusing on the health issue your program is addressing, formulate a methodology that will provide meaningful data that can be used to better solve the health issue within the scope of your organization.
In your assignment,
Refine your Week 2 journal effort.
Define the purpose of the evaluation from the organization’s perspective.
Generate a table of the evaluation stakeholder workgroup (ESW).
Some stakeholders will be represented in the ESW, and some will not. The ESW comprises a total of six to eight members who have a larger stake, or vested interest, in the evaluation findings and can benefit most directly from the evaluation. These members represent the primary users of the evaluation results and generally act as a consultative group throughout the entire planning process, as well as throughout the implementation of the evaluation.

Select the appropriate stage of development for your chosen public health program (planning, implementation, or maintenance).
Describe why this stage of development is appropriate for your chosen public health program.
Discuss the focus of the evaluation through the lens of the logic model you created.
Display the logic model (revised as needed) from your Week 3 journal.
Explain how your logic model informed your methodology.
Summarize issues related to the credibility of your existing data sources available and how they were addressed.
Describe the indicators and/or performance measures linked to evaluation questions.
Include the evaluation methods grid you created in this week’s Barriers journal (revised if needed).
See Figure 4A on page 23 in the required reading and the Week 4 Tables Template for examples of this grid.
Assess how your methods section will help you to evaluate the effectiveness of your chosen program.
