Using statistical process control and data analysis is useful in my field as a case manager in an acute care setting. For example, In case of management, it is important to monitor patient’s discharges, and readmission rates within 30 days and by using this approach it can be easily used to monitor the patient’s records by extracting the quantitative data from the EHR (employee health records system). It will show the patients pertinent information such as discharge dates, readmission dates, admitting and discharge diagnosis, physician groups and insurance information. By reviewing the data obtained will be able to determine the factors associated with readmission. (Snee & Hoerl, 2004). For example, An elderly surgical patient was Discharge to home with home prescriptions and readmitted within 30 days with wound infections. By utilizing QI study/strategies will better determine a better DC planning and create a better plan of care for patients, With this patient, it would be better to Discharge to a Skilled nursing facility where There would be nurses to provide better wound care, administer proper medication, right time and the best route. Physician visits are more feasible in the SNF rather than to home. 30 days readmission will affect the reimbursement in the hospital and will have a tremendous impact on the HCAPs survey. By implementing this statistical process control and data analytics process will provide better nursing care, proper discharge planning, will reinforce readmission rates in the hospital, and as for leaders, this can be a positive way to promote good and effective leadership by implementing quality improvement.
