a. The attached White Paper provides detailed profiles of 24 successful innovative nursing care delivery models. These profiles were developed as part of a research project conducted by Health Workforce Solutions LLC (HWS) and funded by the Robert Wood Johnson Foundation (RWJF).
b. The HWS and RWJF encourage organizations to peruse these models to 1) find ideas and elements to adopt and adapt for their own patients; 2) encourage and inspire continued innovation in care delivery; and 3) identify and profile new models of care that could be replicated throughout the United States.
