Application: Systems Theory
As noted in the Learning Resources, systems theory provides a meaningful and beneficial means of examining challenges in health care organizations. To do this effectively, however, it is essential to assess all system components, as some may be relatively healthy while others are problematic.

For this Assignment, you apply systems theory to the examination of a problem in a department or a unit within a health care organization. (Note: You may use the same problem you identified for the Discussion as long as it meets the criteria for this assignment.)

To prepare:

•Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).
•Reflect on your organization or one with which you are familiar. Within a particular department or unit in this organization, identify a problem the staff is encountering.
•Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.
•Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.
•Research professional standards that are pertinent to your identified problem.
•Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate. (Depending on the organization you have selected, you may have explored these in the Week 1 Discussion.)

Systems Theory: Communication during Shift Report
To understand a sequence of systems and changes that are needed to be implemented to promote quality of care should be measurable. A systems based practice is a tool which can make healthcare changes promote the quality of care given (Johnson, Miller, & Horowitz). Implementing a holistic approach to healthcare for the well-being of the patient is the center of developing a sound comprehensive plan of care.
A problem within our organization is communication. Communication is a key concept that assists nursing personnel with the exchange of information about their patients. Having clear and concise communication skills with members of your healthcare team. The ability for healthcare workers to elaborate and explain conditions of a patient with one another promotes positive patient outcomes
Systems Theory
Within the nursing department of Medical Facilities of America (MFA), the organizational structure is formal and informal. The communication behaviors of the nursing team lack pertinent information between shift reports. The environment during shift reporting takes place at the nursing station; which is an environment of interruption, confusion and loss of train of thought. Moving the shift report away from the nurse’s station will allow for less distraction. Walking rounds allow for staff to discuss care about the patient with less distraction; completing a hand-off report to ensure that all pertinent information about the patient is discussed.
To improve healthcare outcomes of our patients the lines of communication should be open and effective. To provide continuity of care through communication, the entire team which includes the nurse, certified nursing assistants, management staff, social services, therapists, and dietary openly discuss the medical care, emotional, physical, psychosocial, and discharge plans for the patient this done five day or quarterly meetings. Each member of the interdisciplinary team; including nurses and C.N.A.s creating a written or hand typed shift report and sharing this information with one another helps to guide the continuity of care the patient deserves; creating an environment where the all personnel have the same goal in common but from different perspectives. This can be measured by management staff reviewing shift report and nursing documentation. The success of this approach can also be measured by quality indicators and quality measures.
Problem
Communication is a problem that has been identified within the center. Communication between nurses exists during shift change with one another while certified nursing assistants exchange information about patients without the interaction of the nurse. Current shift report between nurses has missing pertinent information. Report is taken between nurses at the nurse’s station amongst multiple nurses, and certified nursing assistants and patients. This creates an environment of interruption, confusion, and loss of train of thought.
Negative communication between healthcare employees can affect the structure of the organization. Communication between nursing staff and others of the interdisciplinary team breaks down when the team no longer functions together. This opens the lines of communication to interpretation, leaving the employee to answer their own questions about patient care creating further facilitating a breakdown of patient care in relation to the goals set by the team and the patient.
An open system gives you the ability to exchanges ideas freely. Ideas can be traded and exchanged to build upon the continuity of care and knowledge of what the patient needs. Open system perspective allows open communication between all parties for the entire organization to strategically plan. Interaction with other people outside of the group, getting ideas and meeting with outside sources shows an open exchange with the environment. The practice of nurses utilizing constant communication between the nurse’s, C.N.A.’s, patients, the patient’s family, the nurse’s co-workers, supervisors, and many others creates a holistic approach to the patients care.

Desired outcome
Decision making through a team approach using an open-system perspective allows for a collaborative treatment plan for the patient. It allows individual members of the team to discuss and identify the physical, psychological, spiritual, and medical needs of the patient. To improve healthcare outcomes of our patients the lines of communication should be open and effective.
Goals and Objectives
To provide continuity of care through communication, the entire team which includes the nurse, certified nursing assistants, management staff, social services, therapists, patient, and dietary openly discuss the medical care, emotional, physical, psychosocial, and discharge plans for the patient. Each party creating a written or hand typed shift report and sharing this information with one another helps to guide the continuity of care the patient deserves; creating an environment where the all personnel have the same goal in common but from different perspectives. The success of this approach can also be measured by quality indicators and quality measures, and patient outcomes, satisfaction.
Policies and Procedures
Licensed nurses are responsible for discussing resident care assignments, condition, activity, and/or follow-up needs with oncoming shift of nursing employees; as well as the overall status of the unit’s operation. The following procedure will ensure the Nursing Policy and Procedure is followed. C.N.A.s coming on duty will report to the unit to obtain an assignment. They will conduct rounds on all their assigned patients to check on them and greet them. The manager of the unit will meet with the current and previous charge nurse to review information about the patients to include pertinent information that discusses the medical care, emotional, physical, psychosocial, and discharge plans for the patient. Licensed nurses and on duty CNAs with the previous licensed nurse will complete rounds reporting to include specific information about the patient. This includes but not limited to: vital signs, weights, showers, IVs, antibiotics, changes in condition, labs, diagnostic test, falls, incident reports, ED visits, and customer concerns.
Professional Standards
Everyone in healthcare is responsible to report changes in condition to other care providers. A “hand-off” communication is standard approach used during shift report. This allows for open discussion for all staff and the time to ask further questions (American Nurses Training Association, 2007). Having open lines of communication about patient care can prevent negative patient outcomes.

Open communication between shift reports with members of the interdisciplinary team improves the working relationship between coworkers and improve patient outcomes. These positive working relationships between healthcare workers allows for the staff to embrace health behaviors in their performance to manage the patients’ health condition whether it is an acute or chronic condition, ultimately improving the quality of care. Improving the quality of care in our patients by the nursing department upholds the mission and values of our organization. Within our organizations mission statement, it is noted to deliver care which will represent quality and value to the patient. The values of our organization state “To work at the highest level of performance, with commitment to continuous improvement” (MFA). Continuous improvement with the center is accomplished by identifying the short-comings of the staff and improving the processes to promote the quality of care we have vision to give.
