Patient MS is a single female who comes in to see you complaining about being tired, easily fatigued, and having trouble concentrating. She is 34 years old and works as a secretary for a law firm. She tells you she is having trouble falling asleep at night, and wakes up too early, and thinks this explains what is happening. She is worried that it takes her longer to do things at work. During the Mental Status Exam, you find she is slightly disheveled, quiet, and slow to answer your questions. She slumps in the chair and looks sad. She describes her mood as fine but spontaneously starts crying, telling you she doesn’t know what is wrong with her, but she no longer enjoys going out with friends. She states she had never felt this way before; in fact, two years ago, she felt the best she had ever felt in her life. She didn’t need much sleep and was so productive at work that she received a raise. She states that at that time, she was never tired like she is now and had multiple boyfriends. She claims this lasted about three months. She currently denies any delusions or hallucinations but admits that she is much more negative about things and isn’t interested in dating. She denies suicidality. She states that the sleep problem started about a month ago and seems to be worse over time.
