It is important for nurses who are transitioning to FHTs (Family Health Teams) to have resources that can support change. This is founded on the fact that in FHTs, there are multiple health disciplines that collaborate in offer team-based care (Patterson et al., 2013).

When nurses are transitioning to interprofessional care, carefully planned strategies are necessary. These strategies should be based on a comprehension of the processes and determinants that influence interprofessional practice and education (Patterson et al., 2013). During change, one of the most commonly experienced barriers is coping with resistance from professionals who are not ready to change. Dealing with resistance requires the provision of supportive resources including Interprofessional Resource Centers (IRCs) as well as embracing communication initiatives via meetings. It is also important to have a facilitator for planning the change.

When an organization wants to engage in change, interprofessional education and practice are among the key components. An organization’s efforts can greatly bear positive impacts if there is use of IRCs (Patterson et al., 2013). In this regard, the six steps that should be adhered to are as follows; preparing and promoting a supportive environment, ensuring that all the members are committed to organizational champions, a thorough examination of the patient care services. Interprofessional transformation, coming up with preceptorships, and finally, evaluation. Nursing leaders and teams that are considering interprofessional education and practice need to review the mentioned six steps in a stepwise fashion so as to direct their development appropriately (Patterson et al., 2013).

Nursing leaders have more power in all nursing teams. Therefore, they can be used to influence change in an organization. Embracing change becomes much easier if the top management in an institution influences it. However, the change should be communicated to all members before being implemented.
